This policy covers this website only. How your health information is handled in treatment is described in the practice's Notice of Privacy Practices, which you receive as a client and can request at any time.

What this website collects

Contact form. If you use the contact form, the site collects what you enter: your name, email address and/or phone number, your preferred contact method and appointment format, and your message. It is used only to reply to you.

No analytics or advertising. This site does not use analytics, advertising, or tracking cookies, and it loads no third-party scripts. Our hosting provider keeps standard server logs (such as IP address, browser type, and pages requested) for security and reliability.

How form messages are delivered

Form messages are sent as ordinary email to the practice through an email delivery service. A blind copy goes to the practice's website provider, who checks that messages are arriving. Messages are not stored on the website. Retention of the email follows the practice's own records practices.

Because this is ordinary email, please do not include health information, diagnoses, insurance numbers, or payment card details in the form or in emails to the practice.

Sharing

Information you send through this site is not sold or shared for marketing. It is shared only with the service providers that host the site and deliver its email, or when required by law.

Children

This website is not directed to children under 13 and does not knowingly collect their information.
